To evaluate eligibility for NeuralCIM treatment, the clinic's medical team requires the following documentation:

Amyloid PET Scan

Imaging scan showing amyloid plaque buildup in the brain. This helps confirm Alzheimer's diagnosis and assess disease progression.

Required for Alzheimer's

Neurologist Report

Official diagnosis documentation from a qualified neurologist confirming the patient's condition, stage, and treatment history.

Required for all patients

Recent Blood Work

Current laboratory test results including complete blood count, metabolic panel, and any other relevant tests from the past 6 months.

Additional Medical Records

Any other documentation relevant to the patient's condition, including previous cognitive assessments, MRI scans, medication history, and treatment records.
